RECOMMENDATION:

I recommend approval and signing of this Hospice Ambulance renewal agreement.

CIVIL COUNSEL REVIEW / RECOMMENDATION:

I have reviewed the attached documents and approve as to legal form.


MOTION:

I move that the Board approve the attached Annual Review and Renewal agreement between Emmet County for Emmet County EMS and Vital Care, Inc. d/b/a McLaren Home Care & Hospice to begin October 1, 2019 and terminate September 30, 2020, and authorize the Board Chair to sign the same.
